That error seems to indicate that no fields were added to a form -- or maybe a form wasn't chosen when using that template for a page?

The example site doesn't include the Form Builder extension but you generally just need to create a form in your admin and add a Page to your site using the Form Builder template and choose the form you created from the dropdown in the Content tab.
